For over 50 years, our client has been more than a health center - they’ve been a lifeline for local families and farm workers.  Since their start, they’ve grown into a network of care offering a variety of health services and support to our rural communities.  Now they’re looking for a dedicated LPN to join their committed team!  As part of the clinical care team, you’ll work closely with providers to support patient care, ensure smooth office flow, and provide hands-on nursing services that make a difference in patients’ daily lives.

Job Duties will include, but not be limited to:

• Preparing and rooming patients for provider visits (vitals, med reconciliation, histories)
• Assisting with procedures, administering vaccines and injections, and performing in-house labs
• Managing medication refills and prior authorizations in coordination with providers
• Supporting communication with patients, pharmacies, and other care team members

Salary:  $27–30.50/hour

• Graduate of an accredited LPN program with current NYS license
• At least 1 year of nursing experience preferred
• Strong communication skills, attention to detail, and EMR proficiency
• Ability to work effectively with a diverse patient population
